Antonio Nino Diaz c3cf06f1a3 Standardise header guards across codebase
All identifiers, regardless of use, that start with two underscores are
reserved. This means they can't be used in header guards.

The style that this project is now to use the full name of the file in
capital letters followed by 'H'. For example, for a file called
"uart_example.h", the header guard is UART_EXAMPLE_H.

The exceptions are files that are imported from other projects:

- CryptoCell driver
- dt-bindings folders
- zlib headers

Antonio Nino Diaz 93c78ed231 libc: Fix all includes in codebase
The codebase was using non-standard headers. It is needed to replace
them by the correct ones so that we can use the new libc headers.

dp-arm 82cb2c1ad9 Use SPDX license identifiers
To make software license auditing simpler, use SPDX[0] license
identifiers instead of duplicating the license text in every file.

NOTE: Files that have been imported by FreeBSD have not been modified.

Soren Brinkmann 65cd299f52 Remove direct usage of __attribute__((foo))
Migrate all direct usage of __attribute__ to usage of their
corresponding macros from cdefs.h.
e.g.:
 - __attribute__((unused)) -> __unused

Sandrine Bailleux c17a4dc34c Make CASSERT() macro callable from anywhere
The CASSERT() macro introduces a typedef for the sole purpose of
triggering a compilation error if the condition to check is false.
This typedef is not used afterwards. As a consequence, when the
CASSERT() macro is called from withing a function block, the compiler
complains and outputs the following error message:

  error: typedef 'msg' locally defined but not used [-Werror=unused-local-typedefs]

This patch adds the "unused" attribute for the aforementioned
typedef. This silences the compiler warning and thus makes the
CASSERT() macro callable from within function blocks as well.

Dan Handley bdbfc3c298 Separate out CASSERT macro into own header
Separate out the CASSERT macro out of bl_common.h into its own
header to allow more efficient header inclusion.
